About 25 percent of patients with advanced melanoma received clinical benefit with treatment of the investigation agent ipilimumab. “We think that treatment with ipilimumab gives patients hope that we really have a new treatment that works,” said expert, director of research at the Angeles Clinic and Research Institute,Santa Monica,California.
Ipilimumab is a fully human monoclonal antibody directed against CTLA-4, a key negative regulator of T-cell responses to tumor-associated antigens. Expert said the drug has been used in treating more than 500 patients suffering from advanced melanoma. The drug is being developed by Bristol-Myers Squibb and Medarex, the sponsors of the trial.
In this open-label, multicenter study, 155 patients with advanced, unresectable Stage III or Stage IV malignant melanoma were enrolled. The average age of the patients was 59 years, and 52 percent were men.
About 5.8 percent of the patients achieved reponses and about 27.1 percent attained disease control. The median overall survival, however was 10.2 months and one-year survival was 46.7 percent, expert said.
Patients received ipilimumab induction dosing at 10 mg/kg every recieved maintenance dosing every 12 weeks beginning with week 24.

The addition of the investigation drug elesclomol to a regimen that includes paclitaxel (Taxol) appears so improve the time to disease progression among Stage metastatic melanoma patients.
“Metastatic melanoma is an-aggressive disease, and patients currently have treatment options,” said Expert, associate professor of medicine at the Winship Cancer Center of the Emory University School of Medicine, Atlanta.
Expert demonstrated that treatment with both drugs resulted in a 7.1 month progressive-free survival, while melanoma patients on pactitaxel alone had 1.8 month interval without disease progression.
The difference between the times to progression reached statistical significance at the p=.020 level, Expert said. Expert and colleagues assigned 24 patients to the combination arm of the trial; eight patients were randomized to the paclitaxel arm of the study.
Elesclomol is a novel, injectable, investigation drug candidate that triggers apoptosis in cancers cells. Elesclomol is being developed under a global collaboration agreement bet ween Synta Pharmaceuticals and GlaxoSmithKline. Phase III trials are underway.

Lack of funding still remains the number one reason for the lack of local researches done in the country. Expert says that there are many researches but these are mostly multicenter studies and collaborations with other countries. Though he says that it gives the expert researchers a much- needed exposure, it still may not do well as it is underutilizing the intelligence and talent of expert researchers.
“We would like to develop our own studies and our own novel ideas. We already have a lot of me, too researches. We would like to have researches that are really ‘ours’. But if we don’t have money to fund, how can we do that?” argues Expert.
Because of the lack of government support, Expert shares that researchers had to turn to non- government organizations and pharmaceutical companies for support. He had done a CDC/USAID-funded research on Public-Private Mix Models for directly observed treatment short course (DOTS) of tubercolosis (TB). There is much pride in his voice as he shares that it is one of his most significant translation research by far. “It has a significant impact on how private physicians can contribute to the control of TB in the Philippines.”
Support coming from pharmaceutical companies is existent, Expert discloses. However, he says that it sometimes comes with an entanglements.’He explained,”You are entangled with the company because, one, you are perceived or associated as a part of that company, and two, because once your have experience in using thier drugs, you are most likely to prefer prescribing it over other drugs.”
He adds that though pharmaceutical companies are generally hands-off when it comes to research results, it is more difficult for local researchers to influence the publication of multicenter trials.
Expert shares that apart from funding and support, another challenge that deters the production of researches in the country is the lack of internationally-recognized publications. As expert says, “If it was not published, it’s as if it never happened.” He reasons,”There is no point in doing research when you will not be able to print knowledge that will be shared by the world.”

The experimental compound odanacatib, a cathepsin K inhibitor, appears as effective as zoledromic acid in reducing levels of urinary N-telopeptide in a four-week, randomized, double blind trial.
“We need a better oral drug to treat women with breast cancer who have painful bone metastases,”said expert, associate professor or medicine at University.” This drug can be taken orally and can be taken with or without meals and the patients do not have to sit up or stand for a half hour after taking the pill.”
The ability of the drug to rapidly reduce and maintain reduction of urinary N-telopeptide has led Research Laboratories, Rahway, New Jersey, the sponsor of the trial, to plan Phase III studies with odanacatib, said expert, senior of clinical sciences at Merck.
Expert also said many of his patients who begin treatment on oral bisphosphonates in order to combat breast cancer bone metastases have to abandon the treatment due to the restrictions required for taking the drugs. In the trial, Expert and colleges recruited 43 patients with confirmed metastatic breast cancer and metastatic bone disease. Among the patient population, 29 women received the experimental compound and 14 women received zoledronic acid. Patients received odanacatib 5 mg once a day or were assigned to zolendromic acid in a 4 mg dosage intravenously on the first day of each month-long cycle.
Within one week of treatment, levels of urinary N-telopeptide a marker of bones resorption had a decreased 77 percent in women taking odanacatib. “That was similar to zoledromic acid where the levels fell by 73 percent,”he said.
